How it works
Ice pick scars are narrow, deep channels that extend from the skin surface down to the dermis or subcutaneous fat. TCA at 70–100% concentration, applied precisely into the scar base with a fine-tipped applicator, causes immediate protein coagulation (frosting) and controlled full-thickness destruction of the scar walls. The body then fills the void with new collagen as it heals — transforming the narrow deep channel into a shallower, wider, smoother depression that is then amenable to laser resurfacing or RF Microneedling in subsequent sessions.
What to expect
A topical numbing is applied. The physician applies a tiny drop of TCA precisely into each individual scar using a fine applicator. Immediately, the scar frosts white — this is the chemical reaction occurring. A mild burning sensation lasts 30–60 seconds. The full procedure takes 30–45 minutes for a typical number of scars. Treated scars will appear dark and form small crusts that fall off in 7–10 days.
Downtime and aftercare
Minimal systemic downtime. Individual scar sites form dark crusts for 7–14 days. No picking — let them detach naturally. Redness at sites may persist for 2–4 weeks. Sun protection essential.
