Fat transfer (lipofilling or fat grafting) uses your own fat — harvested by liposuction from a donor site — and carefully injected into the face to restore volume permanently. It offers completely natural results because the material is your own living tissue, and results are largely permanent once the fat establishes its blood supply.

How it works

Fat is harvested through small-cannula liposuction from a donor site (abdomen, thighs, or flanks) under local anesthesia. The harvested fat is processed (centrifuged or filtered) to concentrate the adipocytes and stem cells and remove oils and blood. The purified fat is then carefully injected into facial planes using fine cannulas, distributing fat in micro-droplets throughout the tissue for maximum graft take. Approximately 60–80% of transferred fat permanently survives once a new blood supply is established (over 3–4 months). The remaining fat resorbs — which is why modest overfilling is performed to account for anticipated resorption. Living fat cells also contain adipose-derived stem cells that have regenerative effects on the surrounding skin and tissue.

What to expect

Performed under local anesthesia (and sedation if desired). Donor site harvest takes 20–30 minutes. Fat injection takes 30–60 minutes. Some post-procedure discomfort and significant swelling for the first 2–3 weeks.

Downtime and aftercare

Moderate-significant. 2–4 weeks of social downtime. Swelling resolves over 4–8 weeks. Final result at 3–4 months.

Results

Permanent once established. Natural, soft result because it's your own tissue. Improvement in skin quality from stem cell effects often noted. Some touch-up sessions may be needed if graft take is less than expected.
