Break the fibrous bands tethering depressed scars to the surface
Subcision is a minor surgical procedure specifically designed for depressed (atrophic) acne scars — rolling and some boxcar scars — where fibrous tissue beneath the scar pulls the skin surface downward. A specialized needle severs these tethering bands, releasing the depression and triggering natural collagen filling beneath.
How it works
During acne healing, fibrous scar tissue can form between the dermis and the deeper subcutaneous tissue. These fibrous strands pull the overlying skin downward, creating the characteristic 'pitted' or 'rolling' appearance. Subcision uses a hypodermic or tri-beveled needle inserted through a tiny entry point, then moved in a fan-like motion beneath the scar to cut these fibrous attachments. This releases the depression immediately and creates a controlled blood-filled pocket beneath the scar. The pooling blood (or added filler/PRP when combined) acts as a natural scaffold for new collagen deposition, gradually filling the depression from below over the following weeks and months. Subcision is often performed in combination with RF Microneedling or PRP for maximum collagen stimulation.
What to expect
Local anesthetic is injected at each treatment site. You'll feel pressure and movement beneath the skin but no pain. Some immediate lifting of depressed scars is visible. Post-treatment, the treated areas will be puffy and bruised for 1–2 weeks as blood disperses and collagen begins forming.
Downtime and aftercare
Moderate. Significant bruising and swelling for 1–2 weeks. Most patients take a few days off social activities. Full resolution of bruising in 10–14 days. Results continue improving over 3–6 months as collagen matures.
